Understanding Privigen Infusion Protocols
Privigen is a 10% liquid preparation of Immune Globulin Intravenous (Human) (IGIV). Calculating the correct infusion rate is critical for patient safety and to minimize infusion-related adverse reactions.
Dosing & Administration Guidelines
The standard concentration of Privigen is 100 mg/mL (10%). Infusion rates are typically calculated based on the patient's body weight and the specific indication (PID, ITP, or CIDP).
- Initial Infusion Rate: Usually started at 0.5 mg/kg/min (0.3 mL/kg/hr).
- Maintenance Rate: If well-tolerated, the rate may be gradually increased to a maximum of 8 mg/kg/min (4.8 mL/kg/hr).
- Titration: It is standard clinical practice to increase the rate every 15-30 minutes as tolerated by the patient.
Example Calculation
For a patient weighing 70 kg receiving a dose of 0.4 g/kg at a target maintenance rate of 4 mg/kg/min:
- Total Dose: 70 kg × 0.4 g/kg = 28 grams.
- Total Volume: 28 grams / 0.10 (10%) = 280 mL.
- Rate (mL/hr): (4 mg/kg/min × 70 kg × 60 min) / 100 mg per mL = 168 mL/hr.
